Abstract
Introduction: Dr P P PATIL Stitch© is very safe, easy to apply and highly effective compression-suture technique for prevention and management of atonic postpartum haemorrhage (PPH) and obstetric haemorrhages. It can be used for atonic PPH following vaginal delivery and lower-segment caesarean section (LSCS), excessive first-trimester bleeding associated with uterine atony, and prophylactically during LSCS in selected women at risk.
Aim and Objective: To reduce PPH incidence and prevent maternal deaths associated with uterine atony.
Materials and Methods: Dr P P PATIL Stitch© is a compression suture designed to compress the uterus uniformly along the direction of uterine muscle fibres without disturbing the uterine cavity. It also temporarily compresses the ascending uterine arteries, producing a combined haemostatic effect of compression and decrease in arterial blood flow. Among 1,050 consecutive LSCS cases, the stitch was applied in 134 having predisposing factors for PPH as prophylactic Stitch. Was applied in 18 PPH cases. No PPH was reported in either group. The technique is simple, can be performed in primary or tertiary-care settings, and requires minimal application time and skill. No special instrument or ligature required.
Procedure: Absorbable ligature is fixed anterolaterally over the lower uterine segment, passed posteriorly through the broad ligament to include the ascending uterine arteries, brought over the fundus on opposite side lateral to the midline, and fixed to the anterolateral aspect of lower segment on the opposite side, where it is fixed. The procedure is repeated contralaterally.
Results: The technique prevented PPH and controlled bleeding while preserving future fertility. Its simplicity makes it valuable in every settings.
Conclusion: Dr P P PATIL Stitch© is an easy, safe, and highly effective method to reduce the incidence and mortality associated with atonic PPH. It requires minimal skill. No special ligature or instrument needed. Unlike other Suture techniques, it compresses uterus uniformly without changing anatomy of Uterine cavity and temporarily compresses ascending Uterine arteries.
