Abstract
Objective: To evaluate first-trimester maternal and ultrasound factors associated with placental insufficiency in pregnant patients treated with acetylsalicylic acid and to identify the clinical and ultrasound characteristics of non-responders.
Materials and methods: This was an observational, analytical, retrospective cohort study of consecutive patients evaluated at Luis Castelazo Ayala Gynecology and Obstetrics Hospital 4 (IMSS) between April 2024 and April 2025. Pregnant women who received prophylaxis with 150 mg/day of acetylsalicylic acid before week 16 and who adhered to the treatment were included.
Results: Of the 1,224 pregnant women screened, 220 were classified as high-risk for preeclampsia and received prophylaxis. Of those, 96 experienced treatment failure (preeclampsia or fetal growth restriction), while 124 demonstrated a favorable response. Maternal age was higher in cases (31.1 ± 6.1 vs. 29.4 ± 5.9 years; p = 0.038). There were no significant differences in body mass index or history of preeclampsia. The group with prophylactic failure had an 873 g decrease in birth weight and a 17-fold higher risk of admission to the neonatal intensive care unit.
Conclusions: Failure of prophylaxis with acetylsalicylic acid is not random. It is associated with increased maternal stress and early alterations in placental vascular remodeling, which are detectable as early as the first trimester.
