Risk factors for hydatidiform mole and their association with maternal age and husbands' occupations

Abstract

Background: Molar pregnancy sits at the benign end of the gestational trophoblastic disease spectrum, arising from abnormal proliferation of placental trophoblast. Although incidence varies widely by region, the contribution of maternal age and paternal occupational exposure to disease risk is still not well characterized, especially in low-resource settings.

Objective: To identify risk factors for molar pregnancy, focusing particularly on maternal age and husband's occupation.

Methods: A cross-sectional study was carried out at Rangpur Medical College Hospital between January 2017 and December 2018. Fifty women with confirmed molar pregnancy (Group A) were compared against 50 age- and socioeconomically matched women with uncomplicated term pregnancies (Group B), enrolled through purposive sampling. Data on sociodemographic background, obstetric history, and husbands' occupational exposures were collected using a structured questionnaire and analyzed in SPSS v23.0 using Chi-square tests, with p<0.05 taken as significant.

Results: Maternal age varied significantly between groups (χ²=12.402, p=0.014), with molar cases concentrated at both ends of the reproductive age range (36% under 20 years; 24% between 30–35 years). Women with molar pregnancy were far more likely to be primigravid (82% vs 40%, p<0.001) and to report a prior abortion (80% vs 52%, p=0.003). Long-term oral contraceptive use beyond four years was also more common in this group (34% vs 8%, p=0.011), as was blood group A or AB (64% vs 32%, p<0.001). Notably, husbands of women with molar pregnancy were far more likely to work in physically demanding jobs (84% vs 36%, p<0.001) and to be exposed to soil and dust (66% vs 18%, p=0.023). No significant differences emerged for education, socioeconomic status, residence, or paternal age and smoking status.

Conclusion: Maternal age and husband's occupational exposure — alongside primigravidity, prior molar pregnancy, and blood group — emerge as meaningful risk factors for molar pregnancy. Greater awareness and simple protective measures for at-risk occupational groups could help reduce associated maternal morbidity.
