Title: Analysis of the triage effect of AI technology (TruScreen) for human papillomavirus-positive women

Abstract

Background: Cervical cancer is a preventable malignant tumor. If effective prevention and screening can be carried out at an early stage, the incidence rate and mortality of cervical cancer will be significantly reduced. A new type of cervical lesion screening system, the real-time photoelectric device TrueScreen (TS), has attracted international attention as a new artificial intelligence assisted cervical cancer screening method. It has the advantages of real-time results, simple operation requirements, and objective evaluation.
Purpose: To explore the triage effect of TS for high-risk human papilloma virus (hrHPV) positive patient.
Methods: A total of 179 cases were selected, who were diagnosed as positive by hrHPV test. Area from site 15 to 31 were detected by TS, and the result was compared with pathological result of cervical biopsy.
Results: There were 23 of 179 cases with abnormal pathological result, including 12 cases with cervical intraepithelial neoplasia (CIN) 1, 2 cases with CIN 2, 3 cases with CIN 3, and 6 case with cervical carcinoma. There were 44 cases of abnormal TS results, of which 15 were pathological abnormalities. The consistency between TS and pathological examination was 65.21% (15/23). The sensitivity, specificity, and AUC of TS were 90.9% (95 CI 57.1%-99.5%), 81.7% (95 CI 72.7%-85.4%), and 0.853 (95 CI 74.7%-96.0%) for CIN2 and worse lesions diagnosis, respectively.
Conclusion:  TS has clinical application value in triage of hrHPV positive patients.  TS has high sensitivity and specificity for detecting CIN2 and worse lesions diagnosis, and is expected to become one of the early screening technologies for CIN, especially providing new clinical strategies for grassroots hospitals lacking experienced pathologists or laboratories, economically disadvantaged areas, and underdeveloped and developing countries with difficult patient follow-up.
