Title: A case of ruptured tubal ectopic pregnancy with uterine anomaly

Abstract

Introduction: Abnormal fusion of the Mullerian ducts or failure of absorption of the septum in the early embryonic period leads to malformations which can present with obstetrical disorders such as infertility, recurrent pregnancy loss, malpresentation and premature labor. The prevalence of congenital uterine anomalies among fertile women is reported as 1:200 to 1:600, whereas that of unicornuate uterus with rudimentary horn is more rare (1 in 100,000).
Case summary:  A 23 years primigravida with amenorrhoea for 5 weeks was brought to EMD with giddiness, loss of consciousness and history of fall. She has abdominal pain since 4 days. UPT is positive. Ultrasound showed hematoma in pelvis and reduced diameter of IVC, features showing possibility of ruptured ectopic pregnancy and hence posted for an emergency laparotomy after stabilising her. Per operative findings shows hemoperitoneum ~ 1.5litre with clots ~300ml(100g), left sided ruptured tubal ectopic at ampullary region ~3X3cm with normal left ovary. The right fallopian tube~0.5cm seen as a blunt end(rudimentary horn) and with absent right ovary. Externally uterus is normal. Histopathology shows features of ruptured ectopic tubal gestation. 
Conclusion: Although ruptured tubal ectopic pregnancies are common, but one associated with uterine anomalies are rare. In this case patient neglected her symptoms and presented with hypovolemic shock which is a life threatening condition and she was saved by prompt management. Here, the couple has to be counselled keenly as the only fertility option available for them is IVF conception. The differential diagnosis can be ruptured ovarian ectopic.

Mounika Kuppili is 28 years old, completed her bachelors in Medicine and Bachelors in Surgery from India, currently doing her junior residency in Masters Of Surgery- Obstetrics and Gynaecology at Sree Mookambika Institute Of Medical Sciences, Kanyakumari, Tamilnadu, India. She has over 3 publications and has presented poster at state/national conference. She is an active member among the student wing in the Obstetrics and Gynaecology societies in India.
