Title: Correlation between pain site and intraoperative findings in deep endometriosis surgery

Abstract

Endometriosis affects approximately 15% of women of reproductive age and significantly reduces quality of life due to severe dysmenorrheal symptoms. It is also associated with chronic pelvic pain and infertility. The number of patients with endometriosis has been increasing in recent years with the trend toward later marriage and fewer childbirths. Although drug therapy has become an option for the treatment of endometriosis, surgical treatment is necessary in cases of chronic pelvic pain or when there is a desire to have a baby.
We investigated the correlation between the location of pain on preoperative examination and intraoperative findings in 12 cases of deep endometriosis surgery performed at our hospital since 2022. Subjects were patients with chronic pelvic pain or strong menstrual pain with numerical rating score (; NRS) 7 or higher and deemed eligible for deep endometriosis surgery, and their ages ranged from 28 to 49 years. The surgical types were laparoscopic vaginal total hysterectomy in 7 cases, laparoscopic ovarian cystectomy and adnexectomy in 4 cases, and laparoscopic myomectomy in 1 case. 
There were cases in which there was no adhesion around the sacral uterine ligament or dorsal to the uterus, but there was strong pain in the perineum and near the virgin membrane, confirming the correlation between intra-abdominal nerve travel and symptoms.
We will present the results, including changes in pain symptoms due to menstrual control during the perioperative period and postoperative management, along with intraoperative videos of the subject cases.

Biography

Hanako Kaseki completed her Ph.D. at the age of 42 years from Nippon Medical School, Japan. She is the head of the minimally invasive surgery team of the Gynecology and Obstetrics Department at Nippon Medical School and Chiba Hokusoh Hospital, Japan. She is certified by the Japan Society of Gynecologic and Obstetric Endoscopy and Minimally Invasive Therapy and also the Japan Society for Endoscopic Surgery.
