Acute fatty liver in pregnancy. Case report

Abstract

Background: Acute fatty liver in pregnancy (AFLP) is a rare but life-threatening condition characterized by liver failure usually in the third trimester. Clinical information about this disease in Mexico is limited, due to the few reported or published cases.

Clinical Case: 22-year-old patient, 35.5 weeks pregnant and poor prenatal control. Upon admission to the obstetrics service, she reported non-specific symptoms such as nausea, vomiting, polydipsia and epigastric pain, later jaundice was added. Suspicion of AFLP was initiated due to the presence of 9/15 Swansea criteria. An emergency cesarean section was performed 6 hours after admission due to fetal bradycardia, and she was then admitted to the intensive care unit. Support measures were maintained until the patient's clinical and biochemical recovery.

Conclusions: Awareness, early diagnosis, and a plan for termination of pregnancy are essential for successful treatment, in cooperation with a multidisciplinary care team. The Swansea criteria should be applied as an early diagnostic test to improve therapeutic outcomes.

Biography

Tamara Espinosa Martínez graduated from the National Autonomous University of Mexico (UNAM). Specialist in Obstetrics and Gynecology with accredited postgraduate training in Yucatan, Mexico. She currently works as a specialist physician at the Mexican Institute of Social Security (IMSS). She is the author of peer-reviewed publications in national and international journals, including case reports on Acute Fatty Liver of Pregnancy and Pregnancy-Related Hemolytic Anemia Associated with G6PD Deficiency. She has also presented case reports on Ovarian Hyperstimulation Syndrome and Complete Duplicated Collecting System at international conferences.
