The impact of serum estradiol on mature oocytes and pregnancy outcomes in IVF : Retrospective observational study
Abstract
Objective: The study aims to assess the correlation of serum estradiol levels with the number of mature follicles, oocytes retrieved and mature oocytes and pregnancy rates in ART.
Materials and methods: An observational retrospective study was conducted in 2 years (2023-2024) in Ridge IVF centre, New Delhi, India. 200 women of age 23-40 years undergoing IVF/ICSI were included. Correlation of Serum Estradiol on the day of trigger to the number of mature follicles (size ≥ 14mm), the number of oocytes retrieved, the number of mature oocytes, S.AMH, Estradiol per oocyte and Estradiol per mature oocyte on the day of trigger were assessed. The correlation of FORT and FOI with S.AMH levels and S.Estradiol levels were also assessed.
Results: The S.Estradiol levels was positively correlated with mature follicles, oocytes retrieved and mature oocytes. Estradiol-to-oocyte and estradiol-to-mature oocyte ratios both increase with rising estradiol levels. 105 achieved a positive pregnancy, while 95 did not. The mean S.E2 in the positive pregnancy group was 3444.4 ± 2780.35 pg/mL, compared to 3166.4 ± 2571.35 pg/mL in the negative pregnancy group and was not statistically significant.
Conclusion: The present study demonstrates that serum estradiol (E2) on the trigger day reflects not only follicular quantity but also the functional competency of granulosa cells when interpreted alongside established markers of ovarian reserve and ovarian sensitivity. However, Serum E2 alone did not predict clinical pregnancy, highlighting the need for a composite, physiology based approach.
