"Dr P P Patil Stitch" for the prevention and cure of atonic PPH

Abstract

Introduction: This is an innovative, very easy, safe and highly effective method which compresses the uterus uniformly (without compromising the uterine cavity). Ascending uterine arteries compressed temporarily.


Aims & Objectives: To decrease PPH incidence and prevent deaths.


Materials & Methods: Material - Dr P P PATIL STITCH©. Rapid Vicryl used. Procedure- Ligature is fixed anterolaterally over lower segment. Passed posteriorly through broad ligament on that side to include the Ascending Uterine arteries. Ligature is brought over the fundus posteriorly, little lateral to midline on opposite side. Brought vertically down on lower segment anterolaterally on other side and fixed. Procedure repeated on opposite side. This was applied as-1. Surgical prophylaxis A) during LSCS B) Atonic uterus 2) Cases prone for PPH- i) Overdistended uterus in multifetal pregnancy, polyhydramnios, large baby, ii)Elderly gravida, iii) Multigravida, iv) prolonged labour, v) IVF pregnancy. 3) Patients with medical disorders-i) Anaemia, ii)Thrombocytopenia, iii)Bleeding disorders, iv) Heart disease. B)During 2nd trimester, Rupture uterus with atonicity 2.Therapeutic procedure: a) Atonic PPH after normal delivery & LSCS.


Results: PPH was successfully controlled in post-LSCS and postdelivery cases. Out of 870 consecutive LSCS cases preventive stitch was applied in selective 115 cases. No PPH was there except in 1 case which had not undergone stitch. No uterine Ischaemia, Synechae observed.


Conclusion: DR P P PATIL STITCH is easiest, safe and highly effective methodology for Atonic PPH. Post LSCS PPH incidence is prevented by applying DR P P PATIL STITCH in selective cases. Incidence and mortality of PPH is drastically reduced. Uterus is conserved and further fertility is preserved. It is game changer especialy in low resource setups.

Biography

Phadnisrao P Patil, MBBS, DGO, DMAS, FMAS, is a distinguished obstetrician known for introducing the “Dr. P.P. Patil Stitch,” a compression suture technique for the prevention of postpartum hemorrhage. He has received several notable honors, including the Dr. Sanjay Gupte Award (2023), Best Paper Award at the Karnataka State Conference (2023), and the IMA National Award (2024). Dr. Patil has presented at district, state, national, and international forums and was invited to present at the FIGO World Conference in South Africa in 2025. He currently practices privately in Kavathe Mahankal, Sangli.
